This notice explains what information Integrated Healthcare Management Solutions (“IHMS,” “we,” “our”) collects through ihmsconnect.com, how we use it, and how to contact us about it. It is a plain-English summary, not a legal contract, and may be updated as our intake practices evolve.
Who this applies to
ihmsconnect.com is a marketing and intake site for healthcare referring partners and the general public. This notice applies to information submitted through the website, including the intake form and any direct emails to our intake team.
What we collect
- Intake form fields — referrer name, organization, phone, email, patient name, patient date of birth, insurance plan, service requested, and any free-text notes you choose to include.
- Direct contact — if you call, fax, or email us, the contents of that communication.
- Basic technical data — standard request information (IP address, user agent, timestamps) collected by our hosting provider for security, abuse prevention, and aggregate site analytics. We do not set advertising or tracking cookies.
How we use it
- To follow up on intake submissions and coordinate care.
- To verify insurance eligibility and service fit.
- To communicate with the referrer about the patient’s coordination plan.
- To maintain operational records of intake submissions received.
What we ask you not to send through the public form
The intake form is intended for intake hand-off only. Please do not paste full clinical records, prescriptions, lab results, or other detailed protected health information into the website’s notes field. Our intake team will request the records they need through secure channels — typically the intake fax line, a secure provider portal, or a direct phone call.
How we share it
- With the IHMS intake team and the contracted home health, DME, and caregiver providers coordinating the patient’s care.
- With insurance plans for the limited purpose of verifying eligibility for the requested services.
- With service providers we use to operate the website (for example, our hosting and email-delivery providers), under their standard terms.
- When required by law, regulation, valid legal process, or to protect the safety of a patient, our team, or the public.
We do not sell or rent personal information. IHMS does not offer, pay, solicit, or receive compensation in exchange for patient referrals.
Patient privacy
Patient health information is treated as confidential and shared only with the people coordinating that patient’s care or as otherwise permitted under applicable law. Where IHMS or a contracted provider is acting in a covered-entity or business-associate role under HIPAA, that relationship is documented separately through written agreements; nothing on this page should be read as those agreements.
Retention
Intake records are retained for the period required to coordinate care and to satisfy legal, billing, and regulatory recordkeeping obligations. Marketing-site technical logs are retained for a shorter period as part of standard hosting operations.
Security
The website is delivered over HTTPS. Form submissions are routed to our intake inbox. We use industry-standard administrative and technical safeguards appropriate to the type of information handled, but no system is perfectly secure; please do not submit highly sensitive information through the public form.
Your choices
- You may decline to submit the form and contact us by phone or fax instead.
- You may request a copy of, correction to, or deletion of information you submitted by contacting our intake team.
Children
The website is not directed at children under 13. Pediatric intake submissions are made by an authorized adult (a clinician, case manager, or family member) on the patient’s behalf.
