- Participate in decisions about your care, services, and discharge planning.
- Choose your providers to the extent permitted by your plan and applicable law.
- Receive clear information about services, expected outcomes, and any limitations.
- Have your health information handled with appropriate privacy safeguards.
- Voice concerns or file complaints without fear of retaliation.
- Refuse care, treatment, or services to the extent permitted by law.

Florida regulatory complaints
Patients have the right to participate in care decisions, choose providers, and file concerns through appropriate regulatory channels. For Florida healthcare facility complaints, contact the AHCA Consumer Complaint Hotline at 1-888-419-3456.
Concerns about a licensed provider
Care is delivered by appropriately licensed providers and service partners. If your concern relates to clinical care delivered in your home (for example, by a home health agency or DME company), please also contact that provider directly. Their licensure information and complaint process should be available upon request.
